Thank you for choosing our office for your chiropractic care! Your first visit will include a consultation and exam by the doctor, lasting up to one hour. Your visit may include x-rays and an adjustment. Please wear comfortable, loose clothing for this initial visit.

We ask that you read through all of the paperwork and fill everything out as completely as possible before you arrive for your scheduled visit. If you are unable to fill out the paperwork, please arrive 15 minutes early, as to allow enough time to get your paperwork in order.
We will need to make a copy of your insurance card/cards and a picture ID. If you have any radiology reports or other medical history reports that may help with your treatment, bring those as well.
